Life Including Variable Annuity & Health is a Florida insurance producer license — DFS class 2-15 — issued and regulated by the Florida Department of Financial Services, Division of Insurance Agent & Agency Services under Chapter 626, Florida Statutes. It authorizes a producer to transact life insurance, fixed and variable annuities, and health insurance.

Under §626.112(4), F.S., a producer must hold an active appointment from an authorized insurer to transact that insurer’s business. Appointments may be held individually or through the producer’s agency, and a license lapses if the producer goes 48 months without one.
